Yes, for all three. Mailboxes are addresses on a domain, so one is required. You can use a domain already on your account, point one you hold elsewhere, or search for a new name during setup.
Across different domains, yes — each order is placed for one domain. Within a single domain the mail has to be served by one platform, because a domain points its mail at one place at a time.
